About Us
Fellowship Home Health, LLC is a private for profit Home Health agency licensed in the state of Texas, certified by Medicare and accredited by Community Health Accreditation Program. Fellowship was established in 2006 with the sole purpose in mind to provide exceptional care in a compassionate way to Patients in their homes. Helping our patients regain independence after surgery, injury or illness, with rehabilitation is our goal.
Our commitment to quality is demonstrated through our ongoing continuous performance improvement program, emphasis on staff development intertwined with the clear understanding that our clients/patients well being is the heart beat of Fellowship.
To demonstrate how serious we are about what we do and how we perform we elected to align ourselves with Community Health Accreditation Partner.

Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. CHAP is the oldest national, community-based accrediting body with more than 5,000 agencies currently accredited nationwide. Through “deeming authority” granted by the lefts for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice, and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards.
