What we do...

Home Care Services

We provide services under the direction of your Physician. Our collaborative team approach to home health ensures the care

you receive is driven by best practices which in turn helps us to achieve the best possible outcomes.  We do NOT discriminate in provision of services or employment with respect to age, race, color, religion, military status, gender preference, sex, marital status, national origin, disability or source of payment.

Nursing Visits

Our staff consists of experienced Nurses and Home Health aides who provide skilled and non-skilled services with professionalism, affection and respect.  Every client is unique and has different needs. This is why each client is personally seen by our Registered Nurse who meets with the patient, family and/or loved ones to perform the initial assessment. This comprehensive

evaluation guides us in creating a "Plan of Care" that is individual to each patient. Our staff is then oriented to

the distinctions of the individual and can therefore provide services specific to their needs.

Our Professional Registered and Licensed Nurses provide skilled nursing care, including:

  • assessment of vital signs,

  • treatments, education, medication review,

  • nutrition, and safety evaluations.

Nurses educate on the impact of acute and chronic illnesses such as:

  • diabetes and heart disease;

  • teach self-management techniques for asthma, diabetes, stroke, heart, and respiratory conditions; and

  • provide Instruction and support with wounds, ostomies, and catheters.

In addition, our skilled nurses will arrange for additional services if needed, such as infusion therapy, telehealth, palliative care, behavioral nursing, and coordinate with others on the home health care team.
